I, (Means Patient/customer), hereby consent to the collection, processing, storage, and use of my personal data by Marengo Asia Hospitals and its affiliates, for the purposes of providing healthcare services, including but not limited to diagnosis, treatment, billing, and administrative purposes.

I understand that my personal data may be shared with healthcare providers, insurers, and other relevant parties as necessary for my care. I further understand that I have the right to access, correct, and delete my personal data, subject to applicable laws and regulations.

I acknowledge that I have read and understood the hospital’s privacy policy, which outlines how my personal data will be handled.
